  Reporting An On The Job Injury

All on-the-job injuries, illnesses, or exposures must be reported by the employee to their supervisor and TOSS immediately.

No matter the severity of the incident, the employee must report all accidents, injuries or exposures immediatley, or as soon as they have knowledge of an incident. If hte employee is not able to contact the supervisor the employee should contact the TOSS office or use one ot the emergency phone numbers. There is no reason an incident should go unreported.

If an on-the-job injury or illness requires examination by a physician, the employee must get authorization from TOSS for the examination prior to seeking medical attention. In cases of severe injury, phone authorization will satisfy the requirement. The employee will be given a Request for Medical Treatment form to present to the physician.

Failure to report an incident can hamper the report and investigation of the incident. Failure to report can also cause the employee to lose benefits or hamper the proper payment of benefits and compensation to the employee. Failure to report an incident may be considered grounds for disciplinary action, up to and including termination. The employee is responsible for reporting the injury or accident to TOSS. Notification of the client supervisor will not meet the reporting standard, the employee must contact their employer - TOSS!
